We are enrolled with the following medical insurance providers: Aetna, Cigna, First Choice Health Network, Humana, Kaiser, Medicare, Premera, Regence, United Healthcare, TriWest, and Tricare. As each insurance carrier offers various types of plans, please contact your insurance company to obtain your specific benefit and coverage plan details and network availability.
We accept cash, check, VISA, and MasterCard. At the time of your visit, we require that you place a credit card on file with our office.
For patients without insurance and patients receiving cosmetic services, payment in full is due at the time of your visit.
For patients with insurance, your copayment is due at the time of service. We will bill your insurance directly for your visit. Once your insurance carrier processes your claim, your credit card on file will be charged up to $200 of the balance for that visit. We will then mail you a statement with any remaining balance due.
